Since some desktop platforms (such as MacBook or MacBook Pro) do not retain the information unless you use the official Adobe Reader to save the completed form, it may be easier for you to simply complete the form on your computer, then print it out & fax it to 972-692-0564. Or, you can scan it and email it to cwcfashionshow@yahoo.com. If your computer is operating in a PC environment instead of a Mac environment, saving the form as a PDF should retain the data, allowing you to email the completed form directly as an attachment or use the SUBMIT FORM button at the top left to electronically send the information.
